Alberta Orders Judge-Led Fatality Inquiry After Edmonton Father Dies Waiting for Emergency Care

The Alberta government has ordered a public fatality inquiry into the death of an Edmonton father who died in a hospital emergency room last month after waiting about eight hours to see a doctor.

The Alberta government has ordered a public fatality inquiry into the death of an Edmonton father who died in a hospital emergency room last month after waiting about eight hours to see a doctor.

Prashant Sreekumar, 44, died on Dec. 22, 2025, in the emergency department at Grey Nuns Community Hospital. He had gone to the hospital complaining of chest pain and later died of cardiac arrest while still awaiting medical care.

Hospital and Surgical Health Services Minister Matt Jones announced the inquiry at a news conference Thursday, saying an independent and public review is necessary despite an internal investigation already being completed.

“While system-level improvements are underway, a detailed independent and public review of how this specific case was managed also needs to be undertaken,” Jones said. “We owe that to his family and to all Albertans.”

Jones said he ordered the inquiry after being left with “concerns and unanswered questions” following the internal review, though he declined to specify what those concerns were. He acknowledged that system pressures at Grey Nuns Hospital contributed to the circumstances surrounding Sreekumar’s death.

“I am not in a position to explain to the public exactly why this happened,” Jones said. “There’s a number of contributing factors, no doubt, but that is the job of the fatality inquiry. It would be inappropriate for me to speculate with incomplete information.”

The Alberta Medical Association has renewed calls for the province to declare a state of emergency in health care, noting that Alberta’s 16 largest hospitals are operating at more than 100 per cent capacity. Health officials also acknowledged Thursday that emergency departments are under extreme strain during what they described as an exceptional respiratory illness season.

Craig Gillespie, a lawyer representing Sreekumar’s family, said the inquiry is a step forward but does not ease the family’s loss.

“This doesn’t change what they are experiencing, and the devastating loss they are experiencing now and will experience forever,” Gillespie said.

The fatality inquiry will examine the circumstances surrounding Sreekumar’s death and issue public findings and recommendations aimed at preventing similar deaths in the future.

Following Sreekumar’s death, the province ordered Acute Care Alberta and Covenant Health to conduct a joint internal review after his wife called for an investigation. The Office of the Chief Medical Examiner also completed a separate review.

Officials confirmed that two other people died in the Grey Nuns emergency department on the same day, though they said both were actively receiving treatment at the time. Further details were not provided, citing privacy legislation.

Patrick Dumelie, CEO of Covenant Health, said the internal review produced recommendations to improve care at the hospital, though details were not disclosed.

“We are in 100 per cent alignment and committed to implementing them,” Dumelie said. “We commit to honouring the lives of those lost by responding with seriousness, urgency and accountability.”

At his funeral earlier this month, Sreekumar was remembered as a devoted husband and father of three, known for his strong values and work ethic. He immigrated to Canada from India in 2002 as a teenager, drawn in part by the country’s reputation for a strong public health-care system. One of his first jobs in Canada was within Alberta’s health-care system.
